STEP 1: Open Twitter and log into your account: twitter.com
At the top left corner below the top navigation bar, you will see your profile section which displays your header image (which is currently blue if you haven't uploaded a header image before), your profile image (avatar) and your Twitter username.
STEP 2: Click on the blue header image area to open your profile page where you will be able to edit your profile including the header banner and the avatar image.
On the right side of the page, just below the large header area, you should see the Edit Profile button.
STEP 3: Click the Edit Profile button to go into edit mode.
You should see the page slightly change and activate editing options such as the header photo, profile photo, displayed name, bio, location, website birthday and theme color.
The large header area at the top will now look like a button like the following:
STEP 4: Click on that Add a header photo button and select your header image from your device (computer or mobile).
On the next screen, you can reposition and scale the header image as you wish or keep it as is.
As also mentioned on the options screen, edges of the image may be cropped on some screens with varying sizes. So, make sure that the center of the image is properly aligned.
STEP 5: Once the header image is ready, click the Apply button and then Save Changes.
Your Twitter header image will be saved and displayed on your profile. If you are not happy with how the header looks, you can modify the image and re-upload.
